JOB SUMMARYCoordinates, negotiates, communicates, and manages the care of assigned patient population along a continuum to facilitate achievement of optimal patient outcomes within appropriate LOS and resource utilization parameters.
QualificationsEducation:
Graduate of a school of nursing.
BSN or MSN preferred.
Experience:
Two (2) to Three (3) years recent clinical nursing experience in an acute care setting required.
Three (3) years recent clinical nursing experience in an acute care setting with prior Care Management experience preferred.
License and Certification:
Current unrestricted License in good standing to practice as a Registered Nurse in the State of Connecticut.
Current Basic Life Support (BLS) certification.
Ability to obtain and maintain department specific certifications.
Certified Nurse Care Manager certification preferred.
KNOWLEDGE, SKILLS AND ABILITIESAbility to read and understand oral and written instructions, and effectively communicate information.
Basic computer skills (including opening/reading email, basic internet use).
Working knowledge of third party payors as well as hospitals registration and billing processes.
Knowledge of health care fiscal management goals and strategies including trends and issues in health care reimbursement and knowledge of care delivery system across the continuum of care.
Excellent interpersonal skills necessary to communicate with departments, employees, physicians and managers.
Ability to manage time and meet deadlines in a hectic work environment.
Ability to handle multiple priorities.
Working knowledge of windows based software.
Ability to influence and persuade members of the professional staff.
PROBLEM SOLVING AND DECISION MAKINGAbility to react and perform under strict timelines and/or emergency situations.
Requires the ability to exercise discretion and use good judgment to perform duties promptly and consistently with little direction.
Requires high level of analytical ability in order to gather and interpret information from various sources.
